TSK: Swimmer’s ear - A tale in 4 acts.

10-year-old patient: My ear hurts after swimming last week.

ENT: Have some Ciprodex drops.

Mom: Those drops are too expensive.

ENT: Switch to Cortisporin [weakest but cheapest otic abx drop]

Mom: Her ear is still hurting the next day.

ENT: Switch to oral cipro and oral prednisone and add tylenol with codeine at adult dosing for pain control. [?!]

Mom: We can’t afford all those medications and I’m worried about using those oral medications in my child.

Cranquis: Hey we can use cipro eye drops for outer ear infections, it costs $8… and her pain is responding nicely to ibuprofen, so let’s stop all the oral prescriptions!

Mom: Cool cool.

Shout out to my 80-something health risk assessment patient today. It came up that I was in the LGBTQ community (he saw my ace ring and, thinking it was a wedding ring, asked if I was married), and he said how much he wanted to be able to ask an LGBTQ person questions, because he’d read a lot about it but had never met someone who was LGBTQ in person. I volunteered that I was also trans and had a few minutes if he wanted to know anything. 

He asked the most respectful questions I’ve ever been asked. They were incredibly personal but the way he did it was with such absolutely genuine curiosity- he really, really wanted to know and understand how transition and asexuality worked. I’m so glad I had the few minutes to talk to him and hopefully give him information he can use or at least think about.
