I have another story to share that we can hopefully learn from. No, this is not a jab at doctors. However, if I have any followers who are physicians, please never act like this.

I’m working in the ER now. I came in at 7pm and got report from the day nurse. Our pt had come back from interventional radiology at 17:00, and from that point on, looked like they were knocking on death’s door. The day RN told me they had been paging the doctor since 17:00. It was now 19:20 and we have yet to hear from anyone. (note that we are paging the surgery team who had taken over this patient from the ER doc.) At this point, the patient is pale, thready pulses. pt using accessory muscles, in resp distress, yet they are maintaining their airway and keeping 02 at 99-100% for now. pt is a little tachy, cool, dry, slightly hypotensive. we paged doc again.

At approx 20:05, Dr. X (1st year resident) comes in the room and immediately has “oh shit” look on their face after laying eyes on patient. they then page their chief resident Dr. Y who comes about 10min later with the same look on their face. long story short, pt ended up intubated with a central line, and art line. they were maxed out on 3 pressors and we started a massive transfusion. 

The pt did not have a good outcome, but it’s important to note that everything we did wouldn’t have helped their outcome even if it had been addressed sooner. This was a very sick patient before they came through the doors. The part of this story I am mad about comes when the ICU nurse came to the ER to help transport the patient. She told me that she overheard Dr. Y talking a couple of hours ago to someone about how some nurses in the ER were blowing up their phone and overreacting about a patient. Dr. Y also laughed about how nurses are know-it-alls.

That is unacceptable. In 2015, we should have made it to the point where physicians and nurses have mutual respect for one another and can work together to care for the patient. Luckily, in the hospital I work in, a majority of the doctors appreciate my input, involve me in the decision making process and acknowledge that I am an educated professional. This scenario doesn’t happen often to me, but it does happen enough that I realize the medical team has a long way to go.

For those who haven’t realized it yet, Medicine is an interdisciplinary field. You’d do well to respect and work with the team if you care about your patients.

Every post on reddit is like “my wife left me, so i spent 100000 on a new man cave” and there’s always a storm trooper of some kind in the room. Ive seen this happen so often on reddit that now i associate storm troopers and the entire star wars universe with divorce.

Hi, anon. You won’t have found it in the book, because it’s not in the book. Here’s the very short version: Neil mentioned in a blog post in early 2005 that he and Terry had recently met to catch up, and they had a conversation in which they mused on what Aziraphale and Crowley were up to. Neil only quoted part of what they decided, namely that it involved them being in the South Downs. A number of us on the lower_tadfield LJ community became wickedly curious about what they could possibly be doing. When I learned both Neil and Terry would be doing readings/Q&As in my then-city (Boston/Cambridge) in September that year, and within a week of each other, I snagged tickets to both events. I promised the l_t folks that if I happened to get called on at both Q&A sessions, I’d ask each author the same question. Thanks to my very noticeable ginger/copper hair, I think, I did get called on at both. Terry gave a charmingly rambling answer about the process of writing GO, but didn’t actually answer my precise question (What did you two decide A&C are doing in the South Downs?) Neil, on the other hand, gave a very similar rambling answer, and then paused and ended with, “But no, what we decided they’re doing in the South Downs is sharing a cottage.” TL;DR it’s a response from one of the authors about an idle speculation in a conversation they had, which I then turned around and used as a writing prompt (it’s what became my long series referred to as CoT in popular shorthand, and other fan creators have also followed suit and used it as a prompt in the years since). I have a longer post or two on this somewhere that include links back to LJ from the time; I’m in a rush now, but I’ll link them here later.  They contain more detail than this explanation.

ETA: The Tumblr posts in question are here and here.  I made them when, on two previous occasions, I got similar questions.  It’s been a wild 14 - 15 years, and I’m still sort of blown away that a neat confluence of circumstances, minor in the grander scheme of things, led to such an impact on my fandom presence (and on my writing life, too).
