Me: I’m hoping you can help explain to me the difference between gas gangrene and wet gangrene as was mentioned in the chart notes.

Ortho: Oh yeah, it’s like, super gnarly.

Me: Because if it’s something polymicrobial then your current antibiotic regimen is probably best, but if it’s the same thing as gas gangrene then I’m going to recommend penicillin and clindamycin since penicillin has better activity against those organisms than vancomycin does, and clinda acts synergistically plus helps with the toxin release.

Ortho: Oh yeah! Juice her up for sure!

Me: But if it’s not the same thing I don’t want to narrow the spectrum—

Ortho: No, yeah, I definitely want you to juice her up, for sure, put in that order for me, thanks, you guys are the best.
